Health care resource utilization and costs for care in hip fracture patients in South Korea using the Health Insurance Review & Assessment service database.
Fracture risk rises with age. Among fractures, hip fractures incur high medical costs. Given aging population, it is crucial to assess the disease burden of hip fracture patients. Using HIRA database, patients aged ≥50 years with a hip fracture diagnosis in 2015 were included. First diagnosis date was defined as index date. Health care resource utilization (HCRU) and costs for care were calculated for 6 months after index date who had no records of hip fracture before 6 months of index date. We divided patients to all-cause and fracture-related groups. HCRU was calculated as the number of outpatient and inpatient visits per patient. Costs were calculated as the average costs per patient and per visit. For 6 months, HCRU of outpatient and inpatient in all-cause group was 15, 2.7 per patient, respectively, in fracture-related group 3.2, 1.5 per patient, respectively. Medical costs per patient in all-cause group were 11,242,155 won. Among them, costs related fracture per patient were accounted 68%. The costs per hospitalization was higher in fracture-related group (5,190,883 won) than all-cause group (3,920,524 won) (p<0.001). Inpatient in fracture-related group paid more costs than all-cause group with their costs per patient exceeding half of those in all-cause group. These suggest hip fracture could be a large economic burden. Thus, extra attention to prevent hip fracture is needed.
